I'm assuming you've seen the 40 Year Old Virgin? Well there's a scene where he asks a doctor "Is it true that if I don't use it, I lose it?", in the context of sex, with his penis being "it". It's a seemingly silly question that most people would laugh at, but from his perspective it's serious.
In other words he's just questioning whether or not you're being serious.
