Saying "that's if you survive" implies that there is a strong chance that you might die.
Just like if i said "So you're getting married and your going to have kids in a few years? That's if you don't get a divorce before you have kids". Say that to some who is just getting married and your going to get the response " So you think i'm going to get a divorce?". It implies that you believe they will meet the outcome of what you are saying.
